Authorization card forms allow labor organizations and associations to collect digital signatures for membership authorization, dues deduction, or other formal agreements.
Step 1: Access Online Actions
- Log in to EveryAction
- From the left-hand side bar, click Engagement> Communication Tools> Online Actions
- Or you can use the “Search for a page” field and type “Online Actions”.
- Click "Create New Form"
- Select "Signup" as the form type (authorization cards use the signup framework)
Step 2: Configure the Basics
- Enter a form name that clearly indicates it's for authorization cards
- Add a title like "Membership Authorization" or "Dues Deduction Authorization"
- Set a vanity URL if desired
- Configure basic settings like CAPTCHA and FastAction
- Click "Next" to continue to the Build Page step
Step 3: Add authorization language
- On the Build Page step, add the formal authorization language required by your organization
- This should include:
Clear statements about what the signer is authorizing
Any legal language required by relevant laws or regulations
Terms and conditions of membership or dues deduction
Information about how to revoke authorization (if applicable)
- Consider consulting legal counsel to ensure your language meets all requirements
Step 4: Add signature collection fields
- In the "Questions" section, add fields for collecting a legally binding signature:
- Full legal name (required)
- Electronic signature field (text field with clear instructions)
- Date of signature (typically auto-generated)
- Acknowledgment checkbox with text like "I agree to the terms above"
- Add other required information fields:
- Employee/member ID number
- Date of birth or other verification information
- Employment information
Step 5: Complete and publish your form
- Add any additional required fields specific to your organization
- Configure the Design step to match your branding
- Set up a confirmation page and email with a copy of the authorization
- Preview the form to test the submission process
- Publish your form when ready
- Consider setting up a secure storage system for the authorization records
