Unable to Save Intake Information for Form: Sharing The Joy

When an applicant is done filling out the form and try to submit, they get an error that reads: Uh Oh. There was a problem. We weren't able to save your intake information, but here is a REQUEST ID “XXXXXXXX” for your records. Please try again later.

Initial investigation suggested an issue with the duplicate checks. They hadn’t changed in years u using the form. I tried to change them anyway with no different result.

Hi @Mike Bates! Do you know whether the error happens for every applicant or only some of them?

    Here’s what could be happening:

    1. Duplicate check is matching — Apricot checks for exact matches on first name, middle name, last name, and date of birth. Even a stale or hidden record can trigger this.
    2. Something else is failing — Triggers, required fields, or field permissions can also block a save.

    To figure out which one:

    1. Test with a brand-new name and DOB. If that works, you’ve likely got a duplicate match on an existing record.
    2. If new records also fail, send the Request ID and form link to Apricot Support. They can look up the exact failure in their logs using that Request ID.

It turns out it was the “State” field. My other administrator resolved it with apricot support. We’re good.
