  1. Go to your Online Actions form in your database

1. Click on "Share" or "Publish" to access sharing options

  1. Select the "Embed" tab

  1. Click the "Copy embed code" button to copy the HTML code to your clipboard

Step 2: Create a new page in Weebly

  1. Log into your Weebly site and select "Edit Site"

  1. Click on the "Pages" tab at the top of the interface

  1. Click the "+" button and select "Standard Page" from the dropdown menu

  1. Enter a name for your page

  1. Click "Save" to create the page

Step 3: Add the embed code to your page

  1. Click on the "Build" tab to edit your new page

  1. From the elements sidebar on the left, find and drag an "Embed Code" element onto your page

  1. A box will appear with a prompt to paste your code

  1. Click inside the box and paste the embed code you copied from Online Actions

  1. Click outside the box to save the code

Step 4: Customize the page around your form

  1. Add any additional content above or below your form:

    1. Introductory text explaining the purpose of the form

    2. Supporting images

    3. Additional calls to action

  2. Format the page to match your site's design

  3. Click "Publish" when you're satisfied with the page
