Creating a Running Medication List in Apricot

Hi,

We created a Tier 2 form that allows you to document medication information, such as the medication name, who ordered the medication, frequency, dose, reason for the medication, any side effects, and its effectiveness. Program staff can create one record for each medication taken by a person. This form is linked to a transfer instruction sheet.

We also have another Tier 2 form that allows you to track the day and time a particular medication was taken, including whether the person took the medication, refused it, etc. This form is completed monthly and includes time slots for multiple days (i.e., Day 1, Day 2, … Day 31).
