Documenting Changes of Data Points

Has any nonprofit identified the necessity to document data points that require modification and established a tier 2 form for data change requests? For example, if a client is homeless and experiences a change in their zip code, but the original zip code must remain for contractual compliance, would it be sensible to create a tier 2 form for the data change request? This form could illustrate both data elements and the date of the change. Is there a more efficient method to document this?

Hi Stephanie! We kind of handle this the opposite way you’re describing. The client profile is considered the “live” information about the client and can be updated by any staff member of any program to reflect that client’s current contact info. Programs that need the client’s address at intake to be recorded and not updated during the course of the program have a separate place to document that on a tier 2 for their program only. Typically it’s on an intake form or a “family info” form (tier 2 under the client profile).

Most of our programs are COA acredited so we need to keep record of how information changes in the system. The “record history” feature largely takes care of this so in most cases it’s okay to just update the original record. The only times we have staff enter a new record is if we need to report on a change over time or if that change is part of the client’s case notes / goals towards successfully completing the program.
