Forms Custom Question Report - Issue with Event Participants data

If you go the Edit Columns button on the ride side of the Online Forms Custom Question Report, you’ll be able to select the Home Address column to add it to the report.

I have added custom questions to an Online Form that are not tracked to standard or custom fields and cannot seem to view people’s responses. Any ideas there?

The Online Forms Custom Question Report will display the responses but they aren’t stored on the contact record initially. If you want to add the responses to the contact record, the first column of that report is VANID and can be used to quickly bulk upload the responses as notes.

Hi Peter, So I don’t need to view them on the contact record. I need to view them on form itself. I have an event tonight. I’ve asked if people need childcare. When I do an Online Forms Custom Questions Report, only some of my custom questions are showing. The others are not.

There’s a question: What is your congregational affiliation if any? (it maps to a custom field on the contact record)

There’s a question: Do you have any food allergies or restrictions? (Answers are showing up on my Online Forms Custom Questions Report)

There’s a question: Will you need childcare? (Answers aren’t showing up anywhere that I can find.)

There’s a question: If yes, please indicate the ages of the children. (Optional)  (Answers aren’t showing up anywhere that I can find.)

Any ideas?

I thought maybe nothing was showing up on my Online Forms Custom Questions Report because nobody requested childcare. But I tested it and signed up saying I need childcare. The info is still not seemingly showing up anywhere.

What question: is the childcare question being mapped to a Survey Question or a Custom Field?
