Nonprofit Hub application not processed twice

I am not sure if this is the right forum to ask questions, but I don’t see another way to ask for support. 

We are in the process of submitting a sponsorship request to the Bank of America. Their form requires us to create a profile in the Nonprofit Hub. 

I have tried twice and confirm that I am the person that oversees all of our fundraising activities but the submission has been reject both times. Can you please advise what might be the issue?
